Seminary Use of Information
Phoenix Seminary collects and processes personal data ("Information") from individuals only as necessary in the exercise of the Seminary's legitimate interests, functions and responsibilities as a private, non-profit institution of higher education. For example, Phoenix Seminary also collects and processes Information from individuals who are applicants for employment positions. Information collected from students or student applicants is used to register or enroll persons in the Seminary, manage a student account, provide academic advising, develop and deliver education programs, track academic progress, analyze and improve education programs, recruitment, regulatory reporting, auditing, maintenance of accreditation, and other related Seminary processes and functions. Phoenix Seminary also uses Information to conduct general demographic and statistical research to improve Seminary programs, to identify appropriate support services or activities, provide reasonable accommodations, enforce Seminary policies or comply with applicable laws. Finally, Information may be shared by Phoenix Seminary with third parties who have entered into contracts with the Seminary to perform functions on behalf of the Seminary, subject to the obligation of confidentiality and safeguarding from unauthorized disclosure.

Seminary (“Phoenix Seminary” or “the Seminary”) provides this website [https://ps.edu] and the application portal (“sites”) as a service to students, faculty, alumni, and prospective students. Use of the sites is subject to the terms and conditions set forth below in Phoenix Seminary’s Privacy Policy (“policy”).
Although Phoenix Seminary respects the privacy of this site’s users (or “members” or “visitors”), the Seminary collects information from its website members and visitors. The Privacy Policy governs users’ interaction with the site and users’ registration for and use of the site’s online services. Except as set forth within this Privacy Policy and/or other published guidelines, Phoenix Seminary does not release personally identifiable information about the site’s visitors without their permission.

The Information Phoenix Seminary Gathers
Phoenix Seminary gathers two types of information about the site’s users:

Keep in mind that, while the Seminary encourages all third parties involved to adhere to this site’s Privacy Policy and to otherwise handle personal information in a responsible manner, Phoenix Seminary cannot and does not assume any responsibility for any actions or omissions of third parties, including the manner in which they use information received either from the Seminary or independently. Phoenix Seminary limits its own e-mail contact and solicitations to those users who have not “opted out” of receiving such communications. The solicitations are limited in that they, among other things, will clearly indicate the originator of the e-mail and provide the user with a method of “opting out” of receiving future communications of a similar nature.
Finally, Phoenix Seminary may use individual members’ data to “pre-populate” forms, which are displayed for the purpose of collecting individual data by the Seminary. In no case does pre-populating a form transfer any data to any third party. Only if the user voluntarily requests that such data be transferred will any transfer take place – for example, if/when a user clicks a “submit form” button or other button.
Phoenix Seminary reserves the right to release any and all information contained within access logs concerning any visitor when that visitor is in violation of the Seminary’s published guidelines, or partakes (or is reasonably suspected of partaking) in any illegal activity, even without a subpoena, warrant, or other court order, and to release such information in response to discovery requests, or in response to any circumstance in which the Seminary, in its sole discretion, deems an emergency. Phoenix Seminary cooperates with law enforcement agencies in identifying those who may be using its servers or services for illegal activities. In any action to enforce this agreement, the prevailing party will be entitled to costs and attorneys' fees. In the event that any of the provisions of this agreement shall be held by a court or other tribunal or competent jurisdiction to be unenforceable, such provisions shall be limited or eliminated to the minimum extent necessary so that this agreement shall otherwise remain in full force and effect and enforceable.
This agreement constitutes the entire agreement between the parties hereto pertaining to the subject matter herein, and any and all prior or contemporaneous written or oral agreements existing between the parties hereto are expressly canceled. No waiver by either party of any breach or default hereunder shall be deemed to be a waiver of any preceding or subsequent breach or default. Any heading, caption, or paragraph title contained in this agreement is inserted only as a matter of convenience and in no way defines or explains any paragraph or provision herein.
Phoenix may at any time change the site, including eliminating or discontinuing any content or feature of the site, restricting its availability or limiting the amount of use permitted. Such changes, modifications, additions, or deletions shall be effective immediately upon notice thereof, which may be given by any means including, but not limited to, posting on the site, or by electronic mail, or by conventional mail. By proceeding with registration, you are consenting to conduct transactions and enter into agreements electronically, with no paper confirmation of said transactions/agreements to be provided. These transactions/agreements may occur at any time during your academic career and may include but are not limited to any of the below stated items. Phoenix Seminary may provide communications and engage in transactions with you in paper form at any time, despite this consent. This includes receiving all communications via the institution’s primary method of contact, which includes but is not limited to personal email account, via the student portal, PS.com or by phone/text to any active phone number provided by me in my student record. Communications may include the following:
I specifically consent to receive my year-end 1098T tax form electronically via my student portal unless I have taken the appropriate measures to opt-out of electronic delivery, in which case a paper copy will be made available. To opt out and request a paper copy, I must email a notice to: finance@ps.edu
Written notice must include your specific request to opt-out of the electronic delivery option for your 1098T document and verification of your current address on file with the institution. If your address is incorrect, you must update your address information via student your portal prior to submitting this request. My request must be received by Phoenix Seminary no later than October 31st of the tax year for which I wish to elect paper delivery. If I do not fulfill the opt-out requirements by the stated deadline, I will be able to view and print a copy of my 1098T by accessing my student portal. Although the duration of this agreement is indefinite, I may withdraw my consent by following the instructions above. Confirmation of my consent withdrawal will be sent to my personal email address.
In order to conduct business electronically with Phoenix Seminary, you must use a computer with a supported operating system and internet browser, sufficient electronic storage capacity on the computer's hard drive or other data storage unit, a printer that is capable of printing from the browser, and email software.
